HOW TO FILE FOR ARBITRATION WITH AMS

Simply send to AMS a letter (AMS, P.O. Box 30466, Cincinnati, Ohio 45230); a fax (877-763-6360); or an email (info@arbitrationmediation.net) requesting a list of arbitrators which contains the following:  

1)    Your name and address (mailing and/or email), as well
       as the name and address of the representative of the
       other party.

2)    Include a copy of the arbitration clause contained in the
       relevant contract.
       a.     If it is a joint submission to arbitration, include the
               submission agreement.
